Record all cases coming under medical care as follow: Date─name─article no.─rating─diagnosis─treatment─disposition. All Narcotics and Barbiturates dispensed shall be recorded and the Surgeon or Master must sign each entry. Record all Narcotics and Barbiturates dispensed on separate sheet. All medications dispensed must be entered, so that the end of the voyage requisitions for completion of the medical stores shall check with medications dispensed during the voyage. All pertinent forms such as accident and illness forms shall be filled out (5 copies each) and an entry made in the Medical log to this effect. These must be turned in to the Claim Department at the completion of the voyage. All deaths, serious injury or illness should be reported immediately by telegraph, cable or radio to Claim Department. Any unusual happening of a medical nature in addition to being entered in this log shall be reported in an individual communication inn duplicate, from the Master or Ship Surgeon to Main Office. Each sheet of the medical log of a passenger vessel shall be signed by the Ship Surgeon. Freight vessel medical log shall be signed by the Master or the Ship's Officer who treated the patient.
